A Personalized Cancer Therapy Delivered Positive Phase 3 Results; The Next Race Is Picking Better Targets
AUSTIN, Texas, Oct. 26, 2026 (GLOBE NEWSWIRE) -- Biotech Insider News Commentary - An mRNA-based cancer therapy designed around each patient’s own tumor has delivered positive Phase 3 results. On October 22, Merck and Moderna announced detailed results from the Phase 3 INTerpath-001 trial showing that intismeran autogene, given with KEYTRUDA®, significantly reduced the risk of melanoma recurrence or death by 42% compared with KEYTRUDA alone. Moderna’s shares still fell 17.57% that day. Beneath the trading is a scientific result that matters for the whole field: a therapy built around immune targets selected from a patient’s own tumor can change outcomes. The open question now is how well, how fast and how cheaply those targets can be chosen. Companies mentioned in today’s commentary include: MindWalk Holdings Corp. (NASDAQ: HYFT), Moderna, Inc. (NASDAQ: MRNA), Merck & Co., Inc. (NYSE: MRK), BioNTech SE (NASDAQ: BNTX).
A Positive Result, a Painful Day
The INTerpath-001 trial enrolled 1,137 patients with melanoma who, following complete surgical resection, were randomized 2:1. Alongside the 42% reduction in recurrence or death, the combination significantly reduced the risk of distant metastasis or death by 38% compared with KEYTRUDA alone. The results were selected for Presidential Symposium I at the European Society for Medical Oncology (ESMO) Congress 2026 on October 24 and have been accepted for publication in The New England Journal of Medicine. According to the companies, overall survival data are not yet mature, but a favorable trend has been observed, and the study remains ongoing to further evaluate overall survival and other key endpoints.
“INTerpath-001 is the first Phase 3 study to demonstrate a clinically meaningful improvement in RFS and DMFS over standard-of-care KEYTRUDA in patients with resected melanoma, setting a new benchmark in the adjuvant setting and representing one of the most significant advances in melanoma treatment since the advent of immunotherapy,” said Dr. Dean Y. Li, president, Merck Research Laboratories.
“From the beginning, we believed mRNA technology could make it possible to develop cancer treatments uniquely matched to each patient’s tumor. The INTerpath-001 results mark a defining moment for that vision and show what’s possible when we rethink how medicines are made,” said Stéphane Bancel, Chief Executive Officer of Moderna.
The market reaction told a different story. Moderna closed at $202.63 on October 22, down 17.57% from the prior close of $245.81, while Merck slipped 1.76% and BioNTech 1.77%, according to Stock Analysis data.
Why Target Selection Is the Whole Game
Intismeran is made for one patient at a time. Information derived from the patient’s tumor and normal DNA is used to create an individualized treatment design, informed by selected neoantigens unique to that patient’s cancer, and each therapy consists of a synthetic mRNA coding for up to 34 neoantigens, according to the companies. The therapy can only work if the immune system recognizes the targets it was given, so the choice of which mutations to include sits at the center of the approach.
That choice is a prediction problem. It depends on estimating which protein fragments will be displayed by a patient’s immune molecules, known as MHC, and which will provoke a response. Results across the field show how much is still to learn.
In August, BioNTech announced that it decided, in consultation with Genentech, to terminate a Phase 2 trial evaluating its individualized mRNA cancer immunotherapy autogene cevumeran as an adjuvant monotherapy in patients with ctDNA-positive, surgically resected colorectal cancer, after the independent Data Safety Monitoring Board identified a numerical imbalance in overall survival between treatment arms. BioNTech said no new safety signals were identified, and its trial in adjuvant pancreatic ductal adenocarcinoma is not affected and continues as planned.
“While this outcome is not what we had envisioned for mRNA as a monotherapy in colorectal cancer, it provides scientific insight into the challenges of treating immunotherapy-insensitive tumor types with immune-suppressive microenvironments and will help inform the further development of investigational mRNA cancer immunotherapies,” said Prof. Özlem Türeci, M.D., Co-Founder and Chief Medical Officer at BioNTech.
Different cancers, designs and combinations produce different outcomes, and the industry is still building the tools to predict immune response earlier.
The Prediction Layer
MindWalk Holdings Corp. (NASDAQ: HYFT), builds those tools for biologics. Its LensAI™ platform produces a composite immune-risk score for protein drugs from three inputs: HYFT®-based humanness scoring, MHC II binding prediction and population allele weighting. MHC binding prediction is the same class of prediction that sits at the heart of neoantigen selection.
On October 21, MindWalk announced a white paper, “Built to Inherit the Curve,” showing that LensAI ran across an entire public antibody database of 890,634 sequences in 4 hours and 41 minutes on Amazon EC2 Hpc8a instances powered by 5th Gen AMD EPYC™ processors. In a study covering 217 therapeutic antibodies cited in the paper, LensAI classified 197 of the 217, approximately 91%, as having an anti-drug antibody incidence risk greater than 30%. The Company notes that the results reflect the specific configurations evaluated and are not representative of all workloads.
“Every antibody drug carries a question: will the patient’s own immune system attack it,” said Dr. Jennifer Bath, President and Chief Executive Officer of MindWalk. “Answering that question late is one of the most expensive failures in this industry.”
MindWalk applies the same approach to its own programs. For its universal dengue program, the Company identified a conserved discontinuous epitope across all four dengue serotypes, and animals immunized with immunogens designed on that predicted target generated antibodies that cross-reacted with antigens of all four serotypes; third-party neutralization testing is underway, according to the Company’s most recent quarterly report. Its universal influenza program is at the research stage, with a HYFT-defined functional constraint confirmed across more than 900 influenza sequence variations. The platform runs on ReefIQ™, MindWalk’s biological context layer, built on HYFT® Technology and a biological representation of 660 million patterns and 25 billion relationships.

What Comes Next
The full intismeran presentation at ESMO and future updates on overall survival will shape expectations for personalized cancer therapies into 2027. As more of these therapies reach late-stage trials, the ability to predict immune response early, across thousands of candidates, becomes a commercial question as well as a scientific one. That is the layer MindWalk is building.
